I noticed that when I create a new mailbox in ADAxes, the resulting mailbox has the correctly formatted smtp address listed in Exchange based on the username, but it also has an smtp address something like this:

&d/City/Domain@CORPEXCH

I've poked around a bit, but I don't see where this is originating from.

Can you post the e-mail generation templates specified for the Exchange E-mail Address Policy that affects that user?

I believe I found the answer to this. We actually don't have a defined email address policy (none that are enabled). With this enabled, the problem goes away.
