Anyone have a process for changing usernames in the AD, sam account name, UPN and the remote routing address in exchange?

No, we usually dont touch these attributes as they are used in many other system as identifier.

In the past some local teams did a change and the mess was endless ...

Hello,

You can use a business rule triggering After updating a user that will make all the required changes only upon the Last Name update. The business rule will look like the following: image.png The process itself can be initiated by a Modify User web interface action with only the Last Name property on the form.
